Мастерство обучения гарантирует правильное использование средств индивидуальной защиты при моделировании клинических встреч с COVID-19
Введение: Правильное использование средств индивидуальной защиты (СИЗ) ограничивает передачу серьезных инфекционных заболеваний медицинским работникам, что критически важно в эпоху коронавирусной болезни 2019 г. (COVID-19).
Однако предыдущие исследования показали, что медицинские работники часто ошибаются при применении и снятии СИЗ. Цель этого исследования состояла в том, чтобы определить, улучшают ли основанное на моделировании, освоение навыков обучения с осознанной практикой правильное использование СИЗ врачами во время имитации клинического контакта с пациентом COVID-19.
Методы:
• Это было предварительное и последующее исследование, проведенное в отделении неотложной помощи крупной академической больницы третичного уровня с 31 марта по 8 апреля 2020 года. В нем приняли участие 117 субъектов, в том числе 56 преподавателей и 61 врач-ординатор.
Перед вмешательством все участники прошли обучение по требованию учебного заведения по использованию СИЗ с помощью онлайн-видео и дополнительных материалов.
• Участники выполнили предварительную оценку навыков с использованием контрольного списка из 21 пункта, в котором указаны шаги по правильному надеванию и снятию СИЗ. Ожидается, что участники получат минимальный проходной балл (MPS) в 100%, определенный группой экспертов с использованием методик установления стандартов Mastery Angoff и Patient Safety. Участники, которые встретились с MPS на предварительном тестировании, были освобождены от образовательного вмешательства.
• Тестирование проводилось до и после личной демонстрации правильной техники надевания и снятия и 20 минут сознательной практики. Первичным результатом было изменение оценок правильного использования СИЗ после нашего образовательного вмешательства.
Вторичные результаты включали различия в показателях успеваемости между преподавателями и врачами-резидентами, а также различия в показателях работы при надевании и снятии.
Результаты. Все участники имели средний балл до тестирования 73,1% (95% доверительный интервал [ДИ], 70,9-75,3%). Результаты предварительного тестирования преподавателей и резидентов были схожими (75,1% против 71,3%, p = 0,082).
Средние баллы за снятие предметов перед тестированием были ниже, чем баллы за надевание, у всех участников (65,8% против 82,8%, p <0,001). Баллы участников увеличились на 26,9% (95% ДИ разницы 24,7-29,1%, p <0,001) после нашего образовательного вмешательства, в результате чего все участники достигли 100% MPS.
Заключение: Вмешательство в овладение мастерством с осознанной практикой обеспечило правильное использование СИЗ субъектами врачей в симулированной клинической встрече с пациентом COVID-19. Необходимо дальнейшее изучение результатов трансляции.

Размышления врачей-интернов об их обучении использованию средств индивидуальной защиты
История вопроса: Текущая пандемия COVID-19 продемонстрировала, что средства индивидуальной защиты (СИЗ) необходимы для предотвращения заражения и передачи инфекционных заболеваний, однако их использование в клинических условиях часто не оптимально.
Обучение и образование важны для обеспечения и поддержания безопасного и эффективного использования СИЗ медицинскими интернами, но существующие методы часто неадекватны для обеспечения соответствующих знаний и навыков.
Целью этого исследования было изучить опыт выпускников медицинских вузов по использованию СИЗ и выявить возможности для улучшения образовательных и учебных программ с целью повышения профессиональной безопасности и безопасности пациентов.
Методы: это исследование было проведено в 2018 году в крупной клинической больнице третичного уровня в Сиднее, Австралия, для изучения собственного опыта использования СИЗ медицинскими стажерами в начале их стажировки.
Рефлексивные группы были собраны сразу после теоретического и практического обучения СИЗ, во время ориентации в больнице. Их опыт студентов-медиков в клинических областях резко контрастировал с рекомендуемой практикой, преподаваемой в больницах, и повлиял на их способность культивировать правильное использование СИЗ.
Выводы: обучение в бакалавриате должно соответствовать передовой практике использования СИЗ и включать практическое обучение, включающее правильные и безопасные методы.
Прогностическая ценность сверхэкспрессии ферментов, связанных с метаболизмом жирных кислот, при плоскоклеточном раке головы и шеи
Перепрограммирование метаболизма подвижных жизненных сил, соответствующего метаболизму липидов, является индикатором плоскоклеточного рака вершины и шеи (SCCHN). Тем не менее, остается неясным, влияет ли экспрессия белка, связанная с окислением жирных кислот (FAO) на выживаемость в SCCHN.
Мы стремились проанализировать экспрессию ферментов, связанных с ФАО, и определить ее корреляцию с клинико-патологическими переменными у пациентов с SCCHN. Возраст и медицинская стадия оставались объективными предикторами выживаемости (скорректированный HR, 1,75; 95% ДИ, 1,22–2,49 для возраста; скорректированный HR, 14,33; 95% ДИ, 1,89-108,60 для стадии III / IV).
Сверхэкспрессия ацил-CoA дегидрогеназы со средней длиной цепи и FAS коррелировала с высшей стадией опухоли (T3 / T4); тем не менее, ни один из этих компонентов не был объективным предиктором выживаемости.

Паклитаксел подавляет миграцию клеток злокачественной глиомы CD133 + U251 за счет снижения экспрессии гликолитических ферментов.
• Жизнеспособное метаболическое перепрограммирование (EMR) позволяет перестраивать последовательность метаболических генов и белков, когда опухолевые клетки адаптируются к своему микроокружению.
EMR характеризуется модификациями метаболического образца и промежуточных продуктов метаболизма, чтобы удовлетворить потребности опухолевых клеток в его или ее злокачественном разрастании и инфильтративном прогрессе.

Разрешение PTX дополнительно ингибировало миграцию клеток. Различия между управляющими и экспериментальными группами были статистически значимыми (P <0,05).
